529. The problem with a lack of timely and reliable lab testing would plague the SARS response. Without a reliable lab test and timely access to results, treating physicians and public health had to diagnose SARS on the basis of clinical presentation and the existence of an epilink. Because the clinical presentation of SARS was similar to so many other diseases, including pneumonia, the epilink became an important part of the diagnostic too. However, as noted throughout this report, as we now know in hindsight, the epilink could not always be identified. It is critical during future outbreaks that lab testing be coordinated and communicated in an effective and timely manner. Non-conventional cyclodextrin complexes It has generally been assumed that the mechanism whereby cyclodextrin exert their effects, especially their augmentation of solubility, is via the formation of noncovalent, dynamic inclusion complexes. This is a model, which regards drugcyclodextrin interactions as discrete phenomenon and ignores the possible interaction of these complexes with one another. It is becoming increasingly apparent that such assumptions may not be universally applicable or all encompassing. Specifically, there is a growing body of evidence that supports the important contribution of noninclusion-based aspects for drug solubilization by cyclodextrins including surfactantlike effects and molecular aggregation. This short review attempts to assess the available literature for areas where such non-inclusion mechanisms are apparent and tries to interpret these in the context of a broader working theory as to how cyclodextrin exert their beneficial effects.

B. TOKOS LICENSURE STATUS The Richardsons also assert that the trial court erred by excluding evidence that Tokos had not obtained a license or certificate of need from the State of Tennessee. They insist that this evidence was admissible under Tenn. R. Evid. 608 because "failure to have appropriate licensing is certainly a factor affecting the credibility of Defendant Tokos in this case." We need not reach the question of the admissibility of this sort of evidence in this case because the Richardsons have failed to demonstrate just how state law required Tokos to have either a license or a certificate of need in order to provide a prescribed medical device to a hospitalized patient on orders of her physician. We are not dealing here with home health care even though the plan, prior to the events of June 27, 1993, had been to discharge Ms. Richardson to permit her to continue her pregnancy at home for ten to fourteen more days. Tokos entered the picture when Dr. Miller ordered an infusion pump while Ms. Richardson was hospitalized. Tokos sent a registered nurse, properly licensed in Tennessee, to the hospital to deliver the pump and to educate Ms. Richardson and the hospital staff in its use.41 The Tokos nurse was not allowed to deliver the pump until the hospital approved her credentials. Even then, she was not allowed to touch Ms. Richardson. The insertion of the needle for the pump was performed by a registered nurse employed by the hospital, and the terbutaline was obtained from the hospital pharmacy on Dr. Miller's orders. From this evidence, we conclude that Tokos was supplying a piece of medical equipment and providing consulting advice to Dr. Miller and the hospital staff regarding the use of its infusion pump and the proper terbutaline dosage to be administered. Ms. Richardson has pointed to no state law or regulation, and our research has failed to discover one, requiring that Tokos obtain a license or certificate of need for its activities in this case and bethanechol. Experiment conducted by Bagnall [8], the intraperitoneal injection of MLT affected neither the development nor the progression of scoliosis. Of note, Machida administered supraphysiologic doses of MLT in mid-light cycle every second day, whereas Bagnall used daily physiologic doses in mid-dark cycle. A second finding which discredits the MLT hypothesis comes from those studies which measured serum MLT in addition to Cobb angles [7, 9, 10, 11, each paper reported low levels of serum MLT in all of the pinx chickens, yet scoliosis developed in only 42-81%. Perhaps low MLT is only one of several factors required for the development of postpinx scoliosis in chickens. In clinical investigations, two studies [14, 15] have reported subnormal levels of MLT in patients with progressive AIS. Others [16, 17, 18, 19] have detected no such relationship. Such negative findings are not grounds for rejecting the MLT hypothesis of AIS, as it may be that none of the patients enrolled in the particular study had low levels of MLT as the cause of their scoliosis. Certainly the latter is impossible to know when the scoliosis is classified as idiopathic! Furthermore, a statistical analysis of clinical studies investigating the etiology of AIS may dilute results which are clinically significant. For example, while Hilibrand [16] failed to detect a significant difference in MLT levels between patients with AIS and controls, it was remarked that 4 patients whose curves progressed after 1 year follow-up had lower morning urine MLT levels relative to controls. Although highly clinically significant, the difference was not statistically significant as a result of the small number of patients with a wide range of values, and confounding by the patients having been braced during the year ie: progression was less than it would have been had bracing not been applied. ; It is difficult to compare the results of the clinical studies mentioned above, for they used different inclusion criteria and measured MLT at different times; inevitably, the result is a large variability in the types of patients studied and the data acquired. Nevertheless, one point has become clear - the importance of enrolling patients of young skeletal age and small Cobb angles who are just developing scoliosis when investigating 10. The World Health Organization cites a contraceptive failure rate of 20% with typical use and 9% with perfect use in nulliparous women. The failure rate for multiparous women in the first 12 months of use of the cap is 40% with typical use and 26% with perfect use.2 and bicalutamide.
