A 56-yr-old, 183-cm, 107-kg male with a long history of progressive low back pain, occasional paresthesias to the midcalf, and radiographic studies demonstrating multiple level spinal stenosis and instability was scheduled for L2-L5 decompressive lumbar laminecfomy with posterior spine fusion, instrumentation, and iliac bone graft. Preoperative electromyography and nerve conduction studies demonstrated no evidence of a myopathy. His history was significant for hypertension, hypercholesterolemia, and palpitations. Medications included sustained-release verapamil240 mg, pravastatin 20 mg, digoxin 0.25 mg, enalapril maleate 5 mg, terazosin hydrochloride 2 mg b.i.d., and terfenadine. Serum electrolytes were normal. The blood urea nitrogen BUN ; was 13 mg dL and creatinine 1.1 mg dL. One preoperative urinalysis was normal while another revealed 3 + glucose, 3 + blood, 2 + ketones, and l + protein with many red blood cells. When there is a reduction in blood pressure, sodium concentration, or renal blood flow, renin is secreted by the kidneys. The renin converts angiotensinogen, which is secreted by the liver, to angiotensin I. Angiotensin I is then converted by angiotensin Iconverting enzyme to angiotensin II. Angiotensin II produces potent vasoconstriction by acting on receptors within blood vessels. It also promotes aldosterone secretion, which causes sodium retention by stimulation of mineralocorticoid receptors in the adrenal cortex. These actions result in increased blood pressure secondary to the vasoconstriction and enhanced cardiac output secondary to sodium retention. The ACE inhibitors inhibit angiotensin Iconverting enzyme, the enzyme responsible for the conversion of angiotensin I to angiotensin II, thus reducing serum levels of this potent vasoconstrictor and aldosterone stimulant. Uses The ACE inhibitors reduce blood pressure, preserve cardiac output, and increase renal blood flow. OCT is OCT1; in the human OCT1 is the main isoform of the liver.9 In humans, hOCT2 has been shown to be expressed at the basolateral side in all three segments of the proximal tubule.10 Other indications for the importance of OCTs in the uptake of cis-platin by proximal tubule cells come from studies with the clone C7 of Madin-Darby canine kidney cells. These cells have been demonstrated to express the isoform 2 of OCT and to be more sensitive to cis-platin toxicity when cis-platin was added to the basolateral versus luminal side.2 This toxicity could be decreased by incubation with cimetidine, a substrate of OCT.2 Because diabetic rabbits are resistant against cis-platin induced nephrotoxicity11 and diabetic rats have a decreased renal expression of OCTs, 1214 it could be speculated that protection conferred by diabetes could be due to a reduced cis-platin uptake in proximal tubular cells as a consequence of OCT downregulation. Although these studies open interesting perspectives, cis-platin transport has never been clearly linked to a specific human transport system or has been investigated in human material preparations. Moreover, interaction of a substance with a particular isoform of OCT does not necessarily predict interaction with other OCTs, because the affinity of these transporters for different substrates are species- and isoform-specific.15 The aim of this study was to investigate whether cis-platin specifically interacts with human OCTs. Acute effects of cis-platin and of its less nephrotoxic derivates oxaliplatin and carboplatin on uptake of the fluorescent organic cation 4-[4- dimethylamino ; styryl]-N-methylpyridinium ASP ; were measured in 1 ; human embryonic kidney cells HEK293 ; stably expressing the hepatic or the renal isoform of OCT hOCT1 or hOCT2 2 ; freshly isolated human proximal tubules; and 3 ; freshly isolated human hepatocyte couplets. Some experiments with human proximal tubules freshly isolated from a diabetic kidney were also performed. To demonstrate that hOCT2 really mediates the uptake of cis-platin, the accumulation of platin in hOCT2-HEK293 and HEK293 cells after 10 minutes of incubation with this anti-cancer drug was also compared. Moreover, induction of apoptosis by incubation of hOCT2-HEK293 cells with cis-platin and its prevention by cimetidine was compared with that observed in nontransfected HEK293 cells. We demonstrate that the transport of cis-platin is isoform- and organ-specific. Moreover, the presence of hOCT2 is necessary for a discrete uptake of cis-platin and to make HEK293 cells sensitive to cis-platin toxicity. These toxic effects are prevented by simultaneous incubation with cimetidine.

Pharmacokinetic Changes Pharmacokinetics refer to the process by which the body acts on drugs to produce effects. This process involves the absorption, distribution, metabolism, and excretion of drugs. Each of these steps can be altered in older persons and ultimately affect the outcomes of medication use. Absorption of medications in the elderly is not significantly altered despite several age-associated changes in gastrointestinal GI ; physiology and function. Increased gastric pH, decreased gastric motility, diminished splanchnic blood flow, and decreased intestinal absorptive surface may exist, but rarely delay the rate or extent of orally consumed drugs 14 ; . However, other situations affecting GI functioning such as surgery, obstruction, and drugs including laxatives, antacids, and anticholinergic agents may clinically alter absorption. Other routes of absorption including the transdermal application of medications may be altered in persons with decreased peripheral blood flow, but the effect of this change has not been clearly defined 15 ; . The distribution of a drug in the body is dependent on several factors including body composition, plasma protein binding, and blood flow to the organs 16 ; . Composition of the aging body changes significantly, resulting in a 10-20 percent decrease in total body water and lean body mass with a proportionate increase in total body fat 17 ; . The volume of distribution of water soluble drugs such as cimetidine, digoxin, aminoglycosides and ethanol is smaller in older adults, which results in increased plasma concentrations and increased risk of toxicity if inappropriate doses are used. Diazepam and chlordiazepoxide are lipid-soluble drugs that experience volumes of distribution. This may result in prolonged duration of action due to the longer elimination half-lives of these drugs. Older adults also tend to have reduced plasma protein binding which is related to a lower serum albumin concentration 18 ; . This decrease may not be clinically significant except in the case of chronic conditions associated with substantial reductions in serum albumin concentrations. The work of many investigators in the past 30 years has clearly established that various dietary factors have marked effects on the metabolism of drugs, environmental chemicals, and certain endogenous substrates. This topic has been reviewed extensively 2-9 ; . However, only recently have we begun to understand some of these effects at the molecular level. Dietary influences on xenobiotic metabolism may alter the therapeutic effects of drugs and the toxicity or carcinogenicity of environmental chemicals. In this article, we review the mechanisms by which dietary chemicals and nutritional status affect the levels and activities of P450 enzymes, xenobiotic metabolism, as well as chemical toxicity and carcinogenicity. Because of space limitations, we have chosen to use selected examples to illustrate key concepts rather than to conduct an exhaustive review on this topic. Pharmacology Amitriptyline inhibits the membrane pump mechanism responsible for the reuptake of norepinephrine and serotonin at the presynaptic nerve terminals, potentiating activity at the postsynaptic terminals. Amitriptyline is one of the more sedative of the tricyclic antidepressants. Half-life averages 21 hours. Therapeutic onset may take up to 30 days. Protein binding equals 94%. Interactions May increase the action of dicumarol. May decrease the effectiveness of clonidine and guanethidine. Increased toxicity may occur when used with fluoxetine. May increase the effects of direct-acting sympathomimetics. May produce toxic effects when used concomitantly with MONOAMINE OXIDASE INHIBITORS. Increased drowsiness occurs when used with alcohol and other CNS depressants. Increased plasma concentrations may occur when used with cimetidine. Precautions Use is contraindicated in patients currently taking monoamine oxidase inhibitors or within 14 days of therapy and during the acute recovery phase following myocardial infarction. Use with caution in pregnancy, patients with impaired liver function, angle-closure glaucoma, urinary retention, increased intraocular pressure, epilepsy, cardiovascular disorders, and in patients with hyperthyroidism. Pregnancy Category D. Adverse Effects Tachycardia, dry mouth, blurred vision, nausea, vomiting, dizziness, weakness, headache, constipation, urinary retention, drowsiness, and weight gain. Patient Consultation Do not discontinue therapy unless otherwise directed. May cause drowsiness. Use care when operating machinery or when mental alertness is required. Results Cumulative dose-response curves to histamine during acute hypoxia Experiment 1 ; Because of the near absence of vascular tone in the normal rat, histamine dilatation was studied in both C and CH rats during preconstriction by hypoxia. Over a wide dose range 1 g to mg ; , histamine caused transitory or prolonged dilatation during hypoxic vasoconstriction in both groups of rats; it had little or no effect on either group during normoxia. Figure 1 compares the effect of 100 g histamine, a dose which caused maximal dilatation, in one C and one CH rat. In each rat, there is prolonged dilatation, but the reduction in hypoxic vasoconstriction is much greater in the CH than the C rat. The figure also shows tachyphylaxis; later in the experiment, after tests with larger doses, a second dose of 100 g has little effect in either rat.
