A number of assessment instruments have been developed to quantify attitudes and beliefs about pain. The 30-item Survey of Pain Attitudes SOPA-B ; has been developed as an alternative to more cumbersome questionnaires. We investigated the possibility of utilizing the SOPA-B to predict appropriate treatment pathways for chronic pain patients. Methods: Pretreatment SOPA-B instruments from 7 patients with chronic back pain were analyzed by an independent investigator blinded to the patients and their case histories, and appropriate initial treatment behavioral vs. medical ; was predicted. Sub-scales of the SOPA-B were identified as having a positive correlation to either medical or behavioral therapies. This prediction was then compared with the case notes and detailed knowledge of patient outcomes by the primary investigator. Results: In all 7 patients, the best response to therapy as judged by activity level, pain rating score, and overall satisfaction ; was achieved with the predicted modality behavioral vs. medical ; . In 2 patients analysis of the SOPA-B suggested the emphasis should have been behavioral initially, when in fact medical therapy was utilized initially and only later did patients undergo behavioral therapy with improvement in their condition. Conclusions: These preliminary results suggest that the SOPA-B may be useful for guiding the initial treatment emphasis in patients with chronic pain. Further investigation of longterm predictive value in a larger number of patients is ongoing, for example, glyburide 3 mg.
