Synopsis A pharmaceutical company complained in June 2004 about the following promotional booklets on Clozril issued to health professionals by Novartis; 'Generic clozapine why take the risk in at risk patients' - January 2004 'Who will pay the price for generic clozapine?' - January 2004 'Clozaril or generic clozapine? You be the judge' - July 2004 The complainant alleged that the materials were inaccurate and unbalanced that they were misleading. On investigation by the MHRA it was found that most of these had already been addressed during inter-company correspondence prior to the involvement of the MHRA. However the complaint was upheld on the point relating to potential consequences from switching to generic clozapine. The booklets were withdrawn and Novartis issued the following corrective statement: `The booklets discuss the relative merits of Cloaril and generic clozapine and give the overall message that there will be negative consequences from switching to generic clozapine both in terms of risk to patients and increased costs. This was considered to be misleading as it was felt that there was insufficient evidence to support these claims. Novartis regrets that the promotional materials had the potential to mislead prescribers. The company remains committed to ethical conduct at all times and takes its responsibility to act in a highly professional manner extremely seriously, with patient safety our top priority.'.

For the management of treatment-resistant schizophrenia, clozapine has superior efficacy to that of typical antipsychotics.1 Recently, its beneficial effects in the management of affective disorders are also being recognized.2 Commonly reported haematological adverse effects of clozapine include agranulocytosis, neutropenia, leukocytosis, and eosinophilia.3 Thrombocytopenia as a complication of clozapine therapy is seldom reported. A review of the database of the Cloaril Patient Monitoring Service CPMS ; of UK and Ireland showed that clozapine was discontinued due to thrombocytopenia in 6 of 6316 patients.4 Apart from this review, only a few case reports5, 6 constitute the existing pool of information regarding clozapineinduced thrombocytopenia. Thrombocytopenia can lead to several complications including dangerous cerebral haemorrhage.7 However, no studies have addressed this adverse effect of clozapine. Therefore, a pilot study was designed to investigate the rate of occurrence, severity, and course of clozapine-induced thrombocytopenia and mebeverine.
Relaxation exercises can be useful in assisting the patient to promote muscle relaxation, improve blood flow to a painful part, as well as to reduce anxiety. Several techniques may be employed including progressive muscle relaxation, rhythmic breathing, and guided imagery. These can be used individually or in combination and should be individually tailored to the patient's needs and preferences. Progressive muscle relaxation involves focusing on individual body parts in an orderly succession e.g. from toes to head. ; The patient is asked to alternately tense and relax each area and to focus on the warmth and relaxation that ensues. Rhythmic breathing involves focusing the patient's attention on the breathing pattern and consciously increasing the depth and slowing the rate of breathing. Guided imagery involves focusing concentration on a situation, feeling, or experience that is pleasant or soothing to the patient. Subject matter should be chosen that the patient has familiarity with and considers to be pleasant and or soothing. Any of these techniques can be enhanced through the use of music or recorded tapes that walk the patient through the process step by step. It is important to choose music or other recordings that the patient finds appealing for them to be successful. Health care professionals are encouraged to try these exercises themselves to become familiar with the techniques and learn first hand of their potential benefits.

Insight into appropriate interventions that lead to sustained weight loss. Additionally, identification of metabolic markers that predict degree of weight loss could be clinically useful in targeting individuals for therapy and establishing achievable goals. Weight reduction in obese individuals with and without diabetes, through diet and behavior modification, is generally correlated with improved insulin sensitivity, glucose tolerance, blood pressure control, and lipid profile 6 8 ; . The majority of studies, addressing the issue of weight loss in patients with diabetes and obesity have demonstrated that relatively small weight loss improves comorbid conditions 9 11 ; . Others have suggested a moderate weight loss is necessary 12 ; . In either case, these studies were of relatively short duration 8, 13 ; . Comparable weight reductions in obese populations with and without diabetes have been noted when employing diet modification or diet in conjunction with behavioral and exercise modification 14 ; . However, a preponderance of previous studies, employing nonpharmacological weight management strategies, note that patients with type 2 diabetes lose less weight than patients without diabetes 13, 15, 16 ; . Reasons for these discrepant results may be related to differences in weight-reduction programs or to physiological differences between those with and without diabetes 1719 ; . An additional reason may be related to the progressive nature of type 2 diabetes. Clinically, as diabetes progresses, therapy must be altered to match these advancing stages. Thus, therapy may be a valid marker of the pathophysiological stage of diabetes. We investigated the magnitude of weight lost by individuals with and without type 2 diabetes as well as relative to type of diabetes therapy, a surrogate marker of disease stage 20 and coreg.
From time to time, registration is sought for a box design with the box being shown in the drawing in its unfolded or knocked-down form and in which all four, or more, side panels and the ends may be viewed at once. This is acceptable provided that the unfolded or knocked-down form is shown in dotted outline and the application contains the statement that the representation of the unfolded container does not form part of the mark, as well as a statement which helps to clarify the actual trade-mark applied for.

TARDIVE DYSKINESIA or WITHDRAWAL DYSKINESIA Dyskinesias are abnormal uncontrollable movements usually involving mouth and trunk but sometimes also the extremities. As an adverse effect of antipsychotic medication, they are thought to be caused by hypersensivity of dopamine receptors in the basal ganglia which results from long term blockade by the antipsychotic medication. Children are more likely to develop withdrawal dyskinesia which arises when the medication is stopped and may last for several months but usually resolves spontaneously. Tardive dyskinesia may occur during treatment with antipsychotic medication. About 15% of schizophrenic patients in longterm maintenance therapy with antipsychotic medication will develop tardive dyskinesia. Decreasing or stopping the medication often worsens the condition, at least temporarily. Increasing the dose of medication often improves the condition, at least temporarily, but no other treatment is currently available. However, most cases of tardive dyskinesia eventually resolve whether medication is continued or not. Risk for tardive dyskinesia increases with increasing doses of medication and length of treatment and is greater for the elderly, women and those with other neurological impairment. NEUROLEPTIC MALIGNANT SYNDROME NMS ; NMS is characterized by extreme rigidity or catatonia, high fever, confusion and disorientation, irregular pulse & or blood pressure and sweating. It may resolve spontaneously, but it can progress to cause death. Studies of how frequently NMS occurs, under what conditions and what governs the risk of reoccurrence have produced a variety of results the answers are not currently clear. ACUTE DYSTONIC REACTIONS Muscle spasms of the tongue, jaw, neck or trunk are typical, often frightening but rarely serious and can be treated or prevented with anticholinergic medications. However, a rare but potentially life threatening variant, laryngeal dystonia, can occur causing respiratory distress. In this case also, anticholinergic treatment should relieve the problem. AGRANULOCYTOSIS This refers to suppression of the production of white blood cells by the bone marrow. This is a rare problem with most antipsychotic medications though it occurs more commonly with Clozaril. SEIZURES Antipsychotic medications lower seizure threshold which may be a problem for people with a predisposition to develop seizures. RETINOPATHY Mellaril at doses over 800 mg day can cause damage to the retina. In children the dangerous dose may be lower. PHOTOSENSITIVITY Increased skin sensitivity to sunlight UV light ; resulting in skin rash. HORMONAL EFFECTS Neuroendocrine effects may result in menstrual irregularities, changes in breast size and function, impotence, Diabetes Mellitus and weight gain. CARDIOVASCULAR RISK The some antipsychotics can produce effects on the heart which interfere with cardiac conduction. This is a particularly significant problem with Mellaril and, less so with Geodon. The following warning was issued by the FDA in 2000.
